By sending this Invoice, I confirm to have taken notice of the below mentioned conditions regarding Export regulations.
I hereby confirm that all information given by me to issue this transport order are correct and to my best knowledge. I also confirm that all loaded items are
properly checked and full functional as requested by the directive 2012/19/EU on waste electrical and electronic equipment (WEEE). Also no item loaded in the
a/m unit violates the regulation (EC) No 1005/2009 of the European parliament and of the council of 16.09.2009 on substances that deplete the ozone layer or
regulation (EC) 1013/2006 of the European parliament and of the council of 14.06.2006 on shipment of waste. Herewith I confirm that the used electrical devices
are functional and that these were checked by a qualified electrician according to the electric law. Record of it (test certificate) can be transmitted immediately
upon request.I understand that I as customer have to take full responsibility if any of the a/m or any other directives and regulation is violated in any case.
